Chronic Care Management

We provide medical and non-medical care coordination post discharge for patients. Using our own clinical team, we’re able to monitor patients to keep the likelihood of being readmitted to the skilled nursing facility/hospital lower.

Wound Care

Use specialty wound services with proven results of healing and wound closure. Services include mist therapies and more for promoting faster healing of wounds.

Cardiology/Pulmonolgy

Given that approximately 67% of hospital readmissions are related to cardiopulmonary decompensations, our cardiology program plays a vital role in reducing patient readmissions and emergency department visits. Many cardiac issues can be effectively managed within skilled nursing facilities, minimizing the need for disruptive hospital transfers. With more than 1 in 5 skilled nursing facility patients having a diagnosis of COPD and 68% of readmissions being respiratory related conditions, it's important to take a proactive approach to pulmonary care as well.
