Upon discharge, you will be contacted by your assigned care coordinator and Primary Care physician. They will assess and design a plan specifically for you to get, and stay, as healthy as possible.

What Does it Cost?

Eligible patients can enroll in this program at no additional cost; insurance verification is required before enrollment, and most major insurance plans are accepted.

How Do You Benefit from Medrina360 Care at Home?

With this program, you will have:

  • A dedicated care coordinator nurse who understands your complete medical history
  • Access to healthcare assistance 24/7
  • Telemedicine with a physician within 48 hours of discharge home
  • On-demand telemedicine visits available as needed
  • Primary Care house calls if needed to address concerns immediately *expanding and not available in every market yet
  • Referrals to Primary Care, Cardiology, and Wound Care physicians, and Physical Medicine and Rehab services as needed
  • Remote Patient Monitoring (RPM) setup and education for you and your family
  • Coordination of care between your pharmacy, specialists, testing centers, hospitals, etc.
  • Help with all your healthcare-related tasks, such as scheduling appointments, requesting refills, and more
  • Help with non-health care related 
    tasks, such as arranging transportation, registration in available community programs, and more
